I, the undersigned, subscriber to the funds of the Melbourne Orphan Asylum, to the amount of   pounds sterling per annum, hereby recommend the undermentioned Child for Admission into said Asylum.

Note - The Committee Meetings for dealing with Applications for the Admission and Removal of Children are held on the third Wednesday in each Month. Applications must reach the Honorary Secretary at least one wekk before the date of the meeting at which they are to be considered.
